Note: You may be able to access your medical records online (subject to GP approval). All coded entries in a patients record will be visible for patients to view, including consultations, BP readings, referrals, etc. Please ask at reception for further information.

1.  EVERY PATIENT AT SUNRISE NOW HAS A NAMED GP - Just ask us to let you know the name of YOUR named GP.

2.  We are closed on Bank Holidays and weekends so please ring 111 for advice and support.

3. Remember to order your medications before any holiday period.  It is also important to have your own supply of over the counter medications in case you need them at night or during weekends

Student Sickness

Student at Nottingham Trent University can advise the University of circumstances which they feel may adversely affect their studies through the Notification of Extenuating Circumstances (NEC) procedures. Please ensure you refer to these procedures by clicking this link NEC Procedures.

We encourage patients to self certify for the first seven days of sickness by using the NTU Self Certification Form

If advice, medication or treatment is required please make an appointment with a doctor or nurse.

DOCTORS WILL NOT NORMALLY GIVE A SICK NOTE FOR MISSING A SINGLE LECTURE OR LAB.

DOCTORS WILL NOT GIVE SICK NOTES TO VERIFY RETROSPECTIVELY OR FOR ANY ILLNESS THAT THEY HAVE NOT TREATED
